Transition Advisor
The Transition Advisor plays a pivotal role in supporting students with additional and complex needs with the intention of reconnecting them with further education and/or training.
The role of the Transition Advisor is to:
- Work in close collaboration with the Careers Advisor, Head Teacher Wellbeing, and School Counsellor.
- Identify "At Risk" and "Disengaged" students in Years 10, 11 and 12 in discussion with Year Advisors & Head Teachers.
- Provide targeted support for all students, particularly those students most at risk from disengaging from their learning.
- Develop Individual Transition Plans to make available post-school options such as full-time employment, post-school preparation for work and study.
- Build partnerships with external agencies to gain specialised support for students including links with local community, TAFE, Colleges, Disability Employment Agencies and Apprenticeship/Traineeship networks.
- Provide one-on-one consultation and develop individualised learning plans to aid students towards achieving personal career goals.
- Facilitate and manage Individual Transition Planning for Life Skills students during years 10, 11 and 12 and linking them with supported post-school employment and/or study.
- Work alongside the Career's Advisor to aid mainstream and Life Skills students in gaining a reality of their desired career pathway through a range of Work Experiences.
- Liaise & work closely with parents and students in their transition process to ensure families have a clear understanding of support and post-school options available for their daughter.
- Facilitate and supervise in-school programs such as Links to Learning.
